Quality Incident Report (QIR) Instructions

Steps to complete the QIR response:

Find the report/QIR number on the file and/or email you received from TFA. Click the "Find Report" button and enter that number.
If the report is found, you will see the incident/report information for the report. If it is not found, you will see a message on your screen and will need to verify the report/QIR number you entered and try again.
The Report Information will show the part in question and discrepant reason for the report, along with a containment action performed (if there was any).
Click/Select the Report Response tab.
- Complete the indicated required fields.
- You may attach any supporting file as to your response.
- When fields have been completed, click the Continue button. The 'Sign Report' tab will display.
Click/Select the Sign Report tab
- Your electronic signature includes your name, email, and job title. (optional)
- Select the check box that you agree that the information you are submitting is correct to the best of your knowledge
- Optionally, you can select the check box to receive an email copy of this response
- When ready, click the Sign Report button
When you are ready to submit your response, select the Submit Response tab and click the Submit button
